Equipment Transfer Request 01/31/17
Use this form when your department has large furniture that needs to be moved within your department or if you have selected items at Surplus that need to be delivered.  If you are having a desk moved, please indicate if it is L shaped.

IT Equipment Removal Request 02/08/17
Use this form when sending anything with a HARD DRIVE to Surplus (i.e. computers, tablets, IPADS, laptops, servers, copiers).  DO NOT include monitors or keyboards on this form, use the Surplus Removal Request form for those items.

Surplus Removal Request 01/31/17
Use this form when your department has furniture or equipment that is no longer used and needs to be removed. If you are having a desk moved, please indicate if it is L shaped.
